Champion Church Graphic Request Form

This form is to be completed for any graphic design needs related to events, promotions, social media, or ministry materials.

Please note the following guidelines to ensure clarity and timely delivery:
  • Submit requests at least 2 weeks in advance of when you need the final graphic. This allows time for design, review, and edits.
  • Be clear and specific with your request. Include all necessary details—event name, date, time, audience, location, and any text or images you want included.
  • One round of revisions is included. Please review carefully before submitting to minimize back-and-forth.
  • Last-minute or same-week requests may not be accepted unless pre-approved by the Marketing Director.
All graphics must align with Champion Church branding, tone, and purpose.
Once submitted, you will receive a confirmation and estimated timeline for delivery.



Important: After submitting this form, please follow up with an email to graphics@championchurch.org that includes all photos, inspiration examples, reference graphics, or logos related to your request.
Please note: Only one round of revisions is included per request. Review your information carefully before submitting to help avoid unnecessary delays.
